Labor migrants in St Petersburg: disease awareness, behavioral
Author/s: Boris Sergeyev, Igor Kazanets, Larisa Ivanova, Irina Zhuravleva, Natalya Isaeva, Tuula Vasankari, Arvid Nyberg, Mikko Vauhkonnen
While there is a number of studies on prevalence of risk behaviors with respect to communicable diseases among labor migrants in Russia, practically no systematic research linking together their socio-economic background, life circumstances, awareness levels and information sources on communicable diseases has been conducted. At the same time in designing disease prevention campaigns, program planners need to take these factors into account. Responding to these demands,…

Abstract
This paper describes an action framework for countries with low tuberculosis (TB) incidence (<100 TB cases per million population) that are striving for TB elimination. The framework sets out priority interventions required for these countries to progress first towards “pre-elimination” (<10 cases per million) and eventually the elimination of TB as a public health problem (less than one case per million). TB epidemiology in most low-incidence countries is characterized…
Read moreIntroducing new tuberculosis diagnostics: the impact of Xpert MTB/ RIF testing on case notifications in Nepal
Author/s: Jacob Creswell, B Rai, R Wali, S Sudrungrot, LM Adhikari, R Pant, S Pyakurel, D Uranw, AJ Codlin
SETTING: The Xpert® MTB/RIF assay is a highly sensitive molecular test with the potential to improve tuberculosis (TB) case detection. However, evidence supporting this potential at a programme level is minimal.
METHODS: Xpert testing following smear microscopy and chest X-ray was implemented as part of routine case finding in 16 districts of Eastern Nepal. Changes in TB case notification were evaluated based on a pre/post analysis, as were expected notifications…

Abstract
Latent tuberculosis infection (LTBI) is characterized by the presence of immune responses to previously acquired Mycobacterium tuberculosis infection without clinical evidence of active tuberculosis (TB). Here we report evidence-based guidelines from the World Health Organization for a public health approach to the management of LTBI in high-risk individuals in countries with high or middle upper income and TB incidence of <100 per 100 000 per year. The guidelines…

Can migration health assessments become a mechanism for global public health good?
Author/s: Kolitha Wickramage, Davide Mosca
Abstract: Migrant health assessments (HAs) consist of a medical examination to assess a migrant’s health status and to provide medical clearance for work or residency based on conditions defined by the destination country and/or employer. We argue that better linkages between health systems and migrant HA processors at the country level are needed to shift these from being limited as an instrument of determining non-admissibility for purposes of visa issuance, to a process…
Read moreResults from early programmatic implementation of Xpert MTB/RIF testing in nine countries
Author/s: Jacob Creswell, Andrew Collins, Emmanuel Andre, Mark Micek, Ahmed Bedru, E Jane Carter, Rajendra-Prasa Yadav, Andrei Mosneaga, Bishwa Raj, Sayera Banu, Miranda Brouwer, Lucie Blok, Suvanand Sahu, Lucica Ditiu
Background: The Xpert MTB/RIF assay has garnered significant interest as a sensitive and rapid diagnostic tool to improve detection of sensitive and drug resistant tuberculosis. However, most existing literature has described the performance of MTB/RIF testing only in study conditions; little information is available on its use in routine case finding. TB REACH is a multi-country initiative focusing on innovative ways to improve case notification.
Tuberculin Skin Tests versus Interferon-Gamma Release Assays in Tuberculosis Screening among Immigrant Visa Applicants
Author/s: Chuke SO, Yen NT, Laserson KF, Phuoc NH, Trinh NA, Nhung DT, Mai VT, Qui AD, Hai HH, Loan le TH, Jones WG, Whitworth WC, Shah JJ, Painter JA, Mazurek GH, Maloney SA
Objective: Use of tuberculin skin tests (TSTs) and interferon gamma release assays (IGRAs) as part of tuberculosis (TB) screening among immigrants from high TB-burden countries has not been fully evaluated.
Methods: Prevalence of Mycobacterium tuberculosis infection (MTBI) based on TST, or the QuantiFERON-TB Gold test (QFT-G), was determined among immigrant applicants in Vietnam bound for the United States (US); factors associated with test results and discordance…
Read moreIntensifying Action to Address HIV and Tuberculosis in Mozambique's Cross-Border Mining Sector
Author/s: Katy Barwise, Andrew Lind, Rod Bennett, and Emilia Martins
Abstract
The southern provinces of Mozambique have some of the world's highest recorded levels of HIV and tuberculosis (TB). They are also characterized by high levels of cross-border migration, particularly to mines in South Africa. Through the Declaration on Tuberculosis in the Mining Sector in August 2012, heads of state of the Southern African Development Community showed an increased commitment to addressing TB and HIV among migrant mine workers, but there is much left to…
Multi-Drug Resistant Tuberculosis in a foreign resident visa holder and implications of a growing inbound migrant flow to Sri Lanka
Author/s: Kolitha Wickramage, S Samaraweera, Sharika Peiris, J Elvitigala, ATND Patabendige
Abstract We present a case of an international labour migrant worker from India who acquired multiple-drug resistant tuberculosis (MDR-TB) as a result of poor treatment compliance throughout his work and travel history. The travel to Sri Lanka was made under the resident visa scheme. Currently there are no mandatory health assessment requirements for inbound migrants such as resident visa holders to Sri Lanka. The diagnosis of MDR-TB was made at a district level chest…
